1. Acceptance

By using this website or booking services you accept these Terms of Service and our Privacy Policy. If you do not agree, do not use the site or book services. Individual services may carry additional consent and intake requirements.

2. Services

We provide hair and barber services, beauty and nail care, skin and brow treatments, spa and bath services, tattoo and piercing, and non-medical wellness programs. Services are provided by licensed or qualified practitioners as required by Utah law and availability.

3. Appointments and Cancellations

• Appointments reserve a station and a practitioner for a set time; please arrive on time — late arrivals may shorten or forfeit the service.
• Cancellations or reschedules should be made at least 24 hours ahead; late cancellations and no-shows may incur a fee or require a deposit for future bookings.
• Group bookings and parties may require a deposit and a signed agreement.

4. Health, Safety and Patch Tests

• Tints, dyes, adhesives, chemical treatments, and permanent cosmetics require a patch test at least 48 hours before the service.
• Disclose allergies, skin conditions, recent treatments, and relevant health information on intake forms — it exists to protect you.
• We may decline or modify services where contraindications, unsafe conditions, or legal restrictions apply.
• Tattoo, piercing, and tanning services require government-issued ID; minors require parental or guardian consent as provided by law.

5. Non-Medical Services

Diet centers, diet workshops, weight loss, and weight reducing services are non-medical lifestyle programs — coaching and structure, not medical advice or treatment. They are not a substitute for a physician, registered dietitian, or mental-health professional. Results vary.

6. Consent Forms

Tattoo, piercing, microblading, permanent makeup, and chemical services require signed consent forms acknowledging procedure risks and aftercare obligations. Forms are retained per licensing rules.

7. Pricing and Payment

Prices are posted in-studio and confirmed at booking; specialty work (color corrections, tattoos, packages) is quoted per consultation. Payment is due at service. Gratuity is at your discretion unless a group policy applies.

8. Refunds and Redo Policy

Services are personal and time-based — we do not offer refunds for completed services. If a service result doesn't match the agreed scope, contact us within 7 days; reasonable adjustments or redo sessions are offered at our discretion. Retail products may be returned unopened within 14 days.

9. Studio Conduct

• Treat staff and other guests respectfully; we may refuse service for abusive or unsafe behavior.
• Children are welcome where the service permits; unsupervised minors are not permitted in wet or treatment areas.
• Follow posted sanitation and safety instructions, especially in sauna, steam, and bath areas.

10. Personal Belongings

Secure valuables; we are not responsible for loss, theft, or damage to personal items except where caused by our gross negligence.

11. Website Use

• Use the site lawfully; do not probe, disrupt, or extract its systems or data at scale.
• The booking form is for genuine appointment requests — no spam or automated submission.
• Do not misrepresent identity, age, or health information in bookings.

12. Intellectual Property

Site content — text, design, graphics, the lash-fan identity — is the property of Jet Black Lashes, LLC and may not be reproduced without permission.

13. Disclaimers

This site is provided "as is." Service descriptions are general; actual services are confirmed at booking. Non-medical services are not health care. We disclaim warranties to the maximum extent permitted by law.

14. Limitation of Liability

To the maximum extent permitted by law, JBL is not liable for indirect or consequential damages. Our liability for services is limited to the price paid for the service at issue, except where law provides otherwise — including liability for negligence in personal care, which is not excluded.

15. Governing Law and Changes

These terms are governed by the laws of the State of Utah; venue lies in the courts of Davis County, Utah, unless law provides otherwise.
